The My Transition Portfolio provides a student portfolio development process that is aligned to the Common Core State Standards (CCSS).  A matrix visual organizer and sample activities highlight the alignment between functional domains and the CCSS.  This Guide has the potential to contribute to improved and empowered outcomes for young adults with disabilities.
